How to Create a Web Form in MS Word XP

  1. Open or create an existing Web page.
  2. From the View menu, select Toolbars, and then click Web Tools.
  3. Place the cursor where the control will be placed.
  4. On the Web Tools toolbar, click the control to be used.
  5. To modify the properties of the controls, select it and then click the Properties button on the Web Tools toolbar. Select the Alphabetic or Categorized tab and then type the desired information in the fields shown.
  6. To allow users to submit the form after filling it out, place a Submit or a Submit with Image control on each Web form.
  7. Click Exit Design Mode on the Web Tools toolbar.
